I am an in-network physician for the following insurance plans:

I am happy to submit claims to other insurance plans, though please be sure that you are familiar with your policy’s out-of-network benefits. I also accept out-of-pocket payment. I do not submit claims to Medicare or OHP.

My Fee Schedule outlines the charges for the procedure codes most often applicable to my services. Please feel free to ask which codes will apply to your visits. I accept cash, check, bank transfer, or credit card (with a 3% processing surcharge). See my Policies & Practice Agreement for additional financial policy details.
